Soft, hard and dual-layer bruxism guards differ first in construction. None of these words guarantees an end to bruxism, pain relief or suitability for every mouth. Selection should be discussed in relation to examination, fit and follow-up.

What comparative studies say
A small older trial with ten participants observed different muscle responses to soft and hard guards. A 2024 review concludes that low- to very-low-certainty data do not show changes in muscle activity or bite force with these splints. These findings do not support a simple ranking. The NIHR review also could not settle the overall comparison of prefabricated and custom devices. A manufacturing difference therefore does not prove superior effectiveness against bruxism. Splints, pain and wear: NIHR review, Riley et al., 2020.
Upper or lower, and what thickness?
Arch and thickness selection are part of the device's prescription or design. They should account for the appliance and your dental situation. The cited studies do not provide a formula for readers to choose independently from a stated thickness.
Report crowns, implants, retainers and any orthodontic treatment. Ask which arch will be covered and how checks will be performed. Do not layer appliances without instructions.
