Orthodontic retention: wires, removable retainers and wear duration

Orthodontic retention maintains tooth position after treatment. It can be fixed, with a bonded wire, or removable, including a clear retainer. It is planned long term and requires checks: no device guarantees perfect stability without follow-up.

Why do teeth need retention?

Tooth position can change after orthodontics. The 2023 Cochrane review assesses devices intended to limit relapse but finds insufficiently certain evidence to identify a superior method in every situation. Retention after orthodontics, Cochrane, Martin et al., 2023.

How long should retention continue?

SFODF recommendations consider 8–10 hours of removable-retainer wear per 24 hours sufficient within the described setting, with a grade C recommendation. This guidance does not replace instructions provided at the end of your treatment. Do not switch schedules independently.

“How many months?” should be supplemented with “how will follow-up be maintained over the years?” Keep the practice's contact details, instructions and appliance information.

If teeth move or the appliance no longer fits

Request a check. A retainer maintains position; it is not for independently realigning shifted teeth. A new impression reproduces the current situation and does not automatically reconstruct the end-of-treatment position.

Common questions

Must a wire stay for life?

No removal date applies to everyone. Retention and monitoring are discussed over the long term; do not remove the wire yourself.

Is a removable retainer better than a wire?

The Cochrane review identifies no overall winner. Some comparisons differ by arch and measured outcome.

Does a bruxism guard also work as a retainer?

Do not assume the appliances are interchangeable. A professional should check how to address both potential needs.
