Functional trainers: purpose, evidence and follow-up in children

A functional trainer is an appliance used in some orthodontic or functional care. Selection depends on the child's assessment. Studies of prefabricated appliances do not support a promise that they will avoid braces.

One name covering several appliances

A prefabricated appliance comes in a standard shape. A custom appliance is made for the patient's circumstances. These categories alone do not explain treatment goals: work on functions, dental guidance or correction of the relationship between the arches. Ask the practitioner for the exact appliance name and expected change.

Two similar-looking appliances are not necessarily interchangeable. A prescription includes a goal, instructions and checks; it is not just the object's shape.

What studies show

The review by Papageorgiou and colleagues finds improvements in some dental measures with prefabricated appliances compared with no treatment. However, several comparisons are less favourable than with custom functional appliances. Certainty ranges from moderate to low.

A 2020 review also identifies limitations in comparisons with activators. This research involves selected, monitored patients. It does not validate choosing a random model for any child.

Distinguish “appliances in this category have been studied” from “this specific product will produce that effect in my child”. No study in the MyoForce scientific evidence file directly tests the Myoforce appliance.

Adherence and tolerance are part of treatment

Actual wear matters: a multicentre trial reports many failures related to poor adherence. This warrants frank discussion during reviews rather than assuming an appliance works simply because it was bought.

Pressure discomfort, sensitivity and speech difficulties have been reported with some appliances. The French review by Amat and Tran Lu Y also highlights unwanted dental effects and encourages scrutiny of manufacturers' claims.

Before starting, plan for three situations: the child refuses to wear it, the appliance causes injury, or it no longer fits properly. The practice should specify what to do. Do not extend wear yourself to “make up” for a missed session.

Questions to ask before buying

  • What problem did assessment identify?
  • Which specific outcome will be assessed, and when?
  • Why does this appliance type suit this situation?
  • Which size and instructions apply to this model?
  • Who checks the fit and tooth movements?
  • What happens if the expected result is not achieved?

Report regular mouth breathing. Mouth-breathing research does not justify presenting an appliance as capable of removing a nasal obstruction. Medical advice may be needed; orthodontic assessment does not replace it. AAPD recommendations on child assessment.

Can it avoid orthodontic treatment later?

The cited studies cannot guarantee this. A functional goal or early correction does not determine the entire orthodontic pathway.

How many hours should it be worn?

There is no duration that applies to every appliance and child. Follow the model's instructions and the recommending practitioner's guidance.
